About Sarah Lilly, PA-C, NP, MSN, BSN, RN

Sarah Lilly, PA-C, brings enthusiasm, expertise, and over a decade of experience to her role as a Physician Assistant at Forum Health Modesto. She has built a distinguished career helping men and women achieve optimal wellness through advanced hormone management.  Known for her meticulous attention to detail and compassionate care, Sarah focuses on understanding each patient’s unique needs to provide personalized, effective solutions. 

After earning her Physician Assistant and Nurse Practitioner certifications from UC Davis in 2009, Sarah gained extensive experience in hormone optimization, skincare, and primary care. Sarah found early in her career that she had a desire to help people prevent chronic diseases and to help them find solutions that were more holistic and integrative. She found a passion in helping people find the root cause through a functional medicine approach.  With certifications in Biote, and Worldlink Medical, she is committed to maintaining the highest standards of safety and care.  

Her work not only supports patients in feeling their best but also helps boost their confidence and overall well-being. Sarah is passionate about building strong relationships with her patients.
